Free COVID-19 Testing this Saturday & Sunday, January 1-2, 2022

Jefferson Middle School Gymnasium – Enter Door #2

Rapid and PCR tests provided by Elite Laboratories and hosted by District 45
BEFORE YOU ARRIVE:
- Be sure to complete the online form prior to your arrival
- The first question on the form will ask you to “Select Client:” Choose OKA-STA (5th from the bottom) so your test goes to the correct lab. Then enter the rest of your information.
- Parents: complete the form for EACH child and/or adult being tested.
- Take a screenshot of your confirmation page. It will include “THANK YOU” with email and date of birth to show technicians when you arrive for testing.
WHEN YOU ARRIVE:
- Enter Door #2. Mask are required for all to enter
- If you see a line out the door, you may wait in car until line is shorter
- Tables will be set up in the gym for testing
- Be sure to remain six feet apart from others
- Test is a saliva mouth swab
Rapid results will be provided via phone call or email within 24 hours, and PCR results will be provided within 48 to 72 hours by email.

ANTES DE LLEGAR:
- Asegúrese de completar el formulario en línea antes de su llegada
- La primera pregunta del formulario le pedirá “Seleccionar Cliente” Elija OKA-STA (quinto desde la parte inferior) para que su prueba vaya al laboratorio correcto. Luego ingrese el resto de su información.
- Padre: complete el formulario para CADA niño y/o adulto que esté siendo examinado.
Tome una imagen de la pantalla de su página de confirmación. Incluirá “GRACIAS” con el correo electrónico y la fecha de nacimiento para mostrarles a los técnicos cuándo llega para la prueba.
CUANDO LLEGUE:
- Entre por la puerta #2. Se requiere cubierta facial para que puedan ingresar.
- Si ve una fila en la puerta, puede esperar en el automóvil hasta que la fila sea más corta.
- Mesas se instalarán en el gimnasio para las pruebas.
- Asegúrese de permanecer a seis pies de distancia de los demás.
- La prueba es un hisopo bucal de saliva.
Los resultados rápidos se proporcionarán por teléfono o correo electrónico dentro de las 24 horas, y los resultados de PCR se proporcionarán dentro de las 48 a 72 horas por correo electrónico.
